Nothing rocket science - just get people to input their choices into a website, so I can use it to spit out all the formatting rather than having to build it all manually in Excel after pulling it out of PMs.

Also, the website should play "Every Rose Has its Thorn" in the background. Potentially not even enabling the submit button until the song's finished.
