My right index finger is beginning to hurt when I use the scroll wheel on my mouse. Feels like the tendons are stretched. Could this be the beginning of carpal tunnel? I hope not. :(

Now that I think about it, the scroll wheel is probably used as a button more than my left mouse button when browsing the web, so my hand naturally hovers over the wheel. Different matter when doing graphics though.

My scroll wheel isn't in the middle. 'Cos it's a trackball. Ball with 3rd/4th fingers, wheel with index, left-click and right-click with thumb.

and will never (if I can possibly help it) use a mouse again. My arm pain is virtually gone too.

I don't even really use the scroll wheel on it either as the page up/down buttons are more convenient.

Another alternative for people suffering from using a mouse is to grab a tablet (like a Wacom) and use that instead. I used to get massive RSI, and although I still suffer a little, my moving towards being far more keyboard-based (shortcuts, screen nav, etc.) and using the Wacom for everything else has helped.

As for expense, I don't see £40–£70 as being remotely expensive if you're buying a piece of durable kit that can essentially stop a condition progressing that can seriously fuck you up for the rest of your life.
